Objective of the Position
- Case Managers are in charge of managing assist and solutions for individuals (recipients) who need assist which includes pregnant women, nursing mothers, adolescent girls, and malnourished children.

Primary duties

- Examines the needs of at risk groups by interviewing people or families to comprehend their health terms, nutrition status, community-related assist, economic hurdles, gender and protection risks.
- Conceptualizes a strategy of action linking at risk people to a health facility, registering children in a feeding programme, referring to psychosocial supportandconnecting families to food assist or livelihood solutions.
- Serves as a bridge between networks and service vendors by referring customers to the right facilities, assisting them comprehend what solutions are accessible and making sure they can entry the service (e.g., organizing transport).
- Visits or calls customers routinely to track development, ensures they are receiving solutions and modifies the care strategy if needed.
- Keeps thorough, private records for each customer and arranges documentation for their supervisors to assist track the effect of the project.
- Works closely with Nutrition Champions, health workers, group voluntary assistants, and local key stakeholders to strengthen the assistance network around the customers.
- Executes group participants’ relation across executing networks.
- Works closely with National Populations Fee to coordinate birth registrations for children under 5.
- Engages with Primary Health Care focal person to organize linkages between group guided programs and the facilities.
- Engages in every month evaluate sessions with other Case Managers/Nutrition Champions.
