Position Overview

We are looking for a very driven and skilled Hospital Delegate to recognize and refer patients with physical rehabilitation needs, which includes stroke, chief injury, and spinal cord injury, for midlevel inpatient admission to our facility. The preferred candidate will possess a robust network of healthcare experts and facilities, clear articulation competencies, and a enthusiasm for supplying premium quality patient care.

Primary duties

• Formulate and sustain connections with healthcare experts, hospitals, and rehabilitation facilities to recognize promise patients with physical rehabilitation needs.
• Acquire and refer patients with terms which includes stroke, chief injury, and spinal cord injury for midlevel inpatient admission to our facility.
• Offer education and information about our facility's solutions and programs to healthcare experts and patients' families.
• Work alongside healthcare teams to guarantee smooth scene changes and continuity of care for referred patients.
• Meet or exceed referral targets and support to the growth of our physical rehabilitation programme.
