Job Description

The Role

  • In this role, you will be a part of the team cohort offering direct (virtual and in-person) self-direction aid offerings to members, who face hurdles with chronic health needs which includes diabetes and hypertension through our creative self-direction model.
  • Your contributions will often be hub and site-based, extending out in the surrounding neighborhood and group but there will also be a primarily virtual part as well. You will be anticipated to follow evidence-based standard procedures for care while explaining to the needs of the members.
  • The Health Mentor will be in charge of members/ individuals and will work in partnership with other members of a virtual team to assist them meet their wellness and health needs.
  • You will be anticipated to address them asynchronously for the most part in addition to setting up concise one on one synchronous chats.
  • The health trainers collaborate with individuals and promotes them to take a leadership position in their wellness by offering them with resources and assist to make the right kinds of lifestyle modifications.
  • Along with the mDoc executive guidance team, you will develop an environment reflecting the values of mDoc Healthcare, which includes respect for our members and team members.

The Health Mentor will:

  • Offer assistance, lifestyle education and direction to participants virtually and in-person and dealing with their queries in addition to inspiring them.
  • Collaborate with the processes and clinical team to comprehend population level data and make modifications to the evidence-based syllabus.
  • Public involvement and mobilization through a multitude of methods
  • Collaborate with locally-driven associates on divided programs
  • Analysis and assemble needed clinical assets.
  • Assist business promotion & sales programs as domain specialist.
  • Evaluate data constantly and offer input within anticipated time frames on enhancements or assist
  • Pay close attention to tracking disease markers
  • Be necessary to use tech for a majority of the work
  • Take consistent clinical and technology evaluations to guarantee she or he aligns with quality and patient-centred concentration

Qualifications

  • Health mentor accreditation preferred
  • MBBS or Registered Nursing officer or pharmacy professional or psychologist or health profile
  • MSC or applicable certification in community-related solutions or health-relevant field preferred (nursing/midwifery, community health, psychology, counseling, health education, exercise science, nutrition and dietetics, etc.) preferred but not necessary
  • Specialized knowledge of inspirational interviewing, reflective listening, conduct adjustment theory, and health psychology preferred
  • At least 3 years of expertise operating in health mentoring, hygiene promotion, chronic illness management or other clinical position preferred.
  • Outstanding verbal and professional writing competencies
  • Capability to actively assist, convey, and train the unique culture and values of mDoc
  • Tech savvy and passionate adopter of work and personal resources. Must be cozy with helping others in using tech and operating remotely
  • Enthusiasm for optimizing health care expertise, results and expenses
  • Interest in analyzing and responding to patient and population health data
  • Fluency in English necessary.
  • Clear expression and "person-centred" competencies
